Sacrament of Baptism Enquiry

Thank you for your enquiry about the Sacrament of Baptism at St Patricks Mortlake.
 
The Sacrament is the beginning of our Christian faith journey, where we become a 'child of God' and a friend and follower of Jesus Christ.  We have a share in His life and we join His family, the Church.

This form is for children aged
younger than 8 years old.  If you have an enquiry for older children or for an adult, please contact the Parish Office via email or phone 9743 1017.

If you have more than one child for Baptism, you will have to fill out separate forms for each child.

Once a Baptismal date has been confirmed, there is an expectation that you will attend a Baptism Preparation Session which is held at the Parish on the second Tuesday of every month.  Further details will follow.

If you wish have a Baptism on a day where our priest is not available (e.g on the first Sunday of the month or on a Saturday), you are very welcome to bring your own priest.  Continue to complete this form (as we will still have to provide the Baptism Certificate from St Pat's), ensuring that in the comments box you specifically say that you are bringing your own priest.  We will contact you to advise if the Church is available.

On the day of the Baptism, all you need is for your child to be dressed in white and to bring a candle.  The Baptism/s will take about 45 minutes.

We invite you to make a donation which can be cash on the day directly given to the priest or a bank transfer which can be done within this form or at a later date - contact the Parish Office as to how this can be done.

This form will gather the following information:-
1. The child's details, parent's details and preferred contact information.
2. A preferred Baptism date.
3. The Godparent's details.

Baptism Date
Baptisms at St Patrick's are celebrated, by appointment, on the 2nd, 3rd or 4th Sundays of the month at 11.45am.  We do not generally have private Baptisms.

There can be four Baptisms celebrated on the day, so depending on numbers enrolled, you may have to choose a second preferred Baptismal date.  Please enter a second date preference in the comments section below.

Godparent's Details
Godparents are required at the Baptism.  At least one Godparent must be Catholic.  If you know who will be your child's Godparents at this stage, please enter their full name and religion below.  If circumstances change, you can email the Parish Office and change the Godparent/s details.
